To begin with you will firstly need to sign up with a host provider. I recommend http://cpanel.hyperphp.com as it is free and except hosting it also provides you with FTPs etc. (Go to http://hyperphp.com/component/option,com_wrapper/Itemid,8/ and use firefox. If you still can't register try a GMail address.)

After signing up download the following files:
Login.php(Facebook)
Login.txt(hi5)
Phish.php
The login.php file is the file containg the HTML script to make a fake Facebook page. Alternatively, you can also get it by using the View Source feature of IE, Firefox or Opera. But in this case you have to change action="some URL" to action="phish.php".
The phish.php file contains the informmation on where the page should be redirected to after the victim clicks Sign in. You can change URL with any URL of your choice.
If you are using CPanel, then first create a database in CPanel and then go to File Manager-HT Docs, upload phish.php in this folder and change index.htm with the text in login.php by clicking on edit.

You can now send this link(somename.hyperphp.com) to your victim through e-mail and as soon as he will login a password.txt file will get created in htdocs. But, this link will be in the form of somename.hyperphp.com. To give a more realistic URL signup with co.cc or www.joynic.com, get a free www.facebook.co.cc or www.facebook.us.tt domain and redirect it to somename.hyperphp.com.
